Removed all traces of fallback.js

It doesn't work very well, it loads very slowly (probably because of connections limit?) and acceptance tests run awfully slow when enabled. It's not usable in its current incarnation, even if the general idea is very good (I wish browsers implemented something like this). Assets are unconditionally loaded from a CDN where possible.
